Social Work Assistant
We are working in collaboration with a Local Authority in Worcestershire to appoint an Adult Social Work Assistant to join their dynamic team!
The role involves providing support to adults aged 18 and over, facilitating their safe transition from acute hospital settings to appropriate care environments. This includes working with individuals with various needs, such as older adults, those with dementia, physical disabilities, mental health conditions, and mild to moderate learning disabilities and/or autism.
Key Responsibilities:
- Utilise a strength-based approach to assist adults in managing their care needs and mitigating risks.
- Collaborate with other professionals to ensure a holistic, multi-agency approach to enable independent living.
- Engage in reflective practice and contribute actively to team discussions.
- Identify and support adults and carers at risk, in coordination with qualified social workers.
- Participate in meetings with adults and carers, contributing to decision-making processes and monitoring progress.
- Conduct eligibility assessments under the Care Act (2014) through meaningful conversations.
