We are working with a client in Southampton is seeking a dedicated Reablement Assessor to deliver strengths-based assessments and coordinate short-term, outcome-focused support. This role focuses on promoting independence, supporting recovery, and ensuring individuals receive timely and effective interventions.

As a Reablement Assessor, you will work closely with individuals, families and partner agencies to assess needs, create person-centred reablement plans, and review progress. You will play a key role in enabling people to regain independence and ensuring safe, effective support pathways.

Key responsibilities:

  • Undertake strengths-based assessments and reviews to identify needs and desired outcomes
  • Develop and coordinate person-centred reablement plans focused on independence and recovery
  • Provide advice, guidance and signposting to appropriate community and statutory services
  • Manage risk and safeguarding in line with Making Safeguarding Personal principles
  • Work collaboratively with therapy teams, health partners and internal colleagues
  • Maintain accurate and timely records while supporting service performance and improvements


Key Requirements:

  • Experience conducting assessments within a health or social care setting
  • Strong understanding of reablement, independence-focused practice, and person-centred care
  • Knowledge of safeguarding principles and risk management
  • Ability to work collaboratively with multidisciplinary teams and external partners
  • Excellent communication, organisational and record-keeping skills



Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £18.72 PAYE
Working pattern - 37 hours per week.
