An exciting opportunity awaits for an Occupational Therapist to join STSS (Short Term Support Services) in Lancashire. STSS is committed to providing a single point of access to community intermediate care services, collaborating with referring professionals to advance initial assessments and deliver relevant services to individuals in need.

The role involves conducting holistic face-to-face assessments, reviews, and support planning, facilitating the transition onto appropriate services, all outside of the hospital setting. Utilising a strengths-based approach, you will ensure individuals receive timely support tailored to their needs, empowering them to achieve independence and lead a fulfilling life on their terms. Embracing the ethos of a reabling approach, you will guide the team towards outcome-focused practices.

Key Responsibilities:

• Conduct comprehensive, strength-based assessments.
• Manage risk effectively to promote a 'home first' mindset.
• Build strong professional relationships with internal and external stakeholders.
• Support discharge processes from hospitals and prevent unnecessary admissions.
• Collaborate with Community Health & Social Care partners.
• Advocate for integrated approaches to benefit service users.

Requirements:

• Demonstrated ability to inspire colleagues through best practices.
• Proficiency in managing risk and promoting positive risk-taking.
• Experience in managing a varied caseload, including complex cases.
• Knowledge of equipment, moving and handling techniques, and single-handed care approaches.
