The primary responsibility of the Occupational Therapist will be to assess and provide interventions for adults and older people requiring specialist housing adaptation, equipment needs, and hospital discharge planning. This includes rehabilitation, health promotion, equipment and adaptations, and maintaining communication with other healthcare professionals.

Key Responsibilities:

  • Perform specialist community Occupational Therapy assessments for disabled individuals of all ages, considering their health, social, psychological, and housing needs.
  • Develop and implement intervention plans aimed at reducing the impact of disability and improving overall well-being.
  • Manage a caseload of challenging needs independently, demonstrating high-level problem-solving skills and autonomy in decision-making.
  • Collaborate with lead practitioners and other healthcare professionals to ensure best practice and holistic care for clients.
  • Maintain accurate documentation and records of assessments, interventions, and progress.

Requirements:

  • Bachelor's or Master's degree in Occupational Therapy and registered with the HCPC.
  • Experience working with adults and older people with diverse physical and mental health conditions.
  • Strong assessment and intervention skills, with a focus on rehabilitation and promoting independence.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team.
  • Understanding of relevant legislation and guidelines related to occupational therapy practice.
