Occupational Therapist - Community Access Team

3-Month Initial Contract with view to extend

£30 plus per hour

I am currently looking for an Occupational Therapist on behalf of my client who is looking for someone to join their Community Access Team operates on a multidisciplinary model, integrating Occupational Therapy and Social Work. Prioritising an OT first approach, aiming to enhance independence and prevent or delay the need for services.

Responsibilities:

Manage a variety of tasks, including addressing complex housing situations, screening for Disabled Facilities Grants (DFG), handling initial enquiries related to moving and handling, utilising reablement services, issuing equipment, performing minor adaptation work, and leveraging technology-enabled care.

Maintain awareness of safeguarding principles and collaborate with the (MDT) to ensure safety and effective risk management.

Requirements:

• Valid Occupational Therapy qualification and HCPC registration.
• Office presence required with the flexibility to work from home intermittently.
• Possession of a driving license and access to a vehicle during working hours.
• Proficiency in conducting a significant portion of work over the telephone.
