Spencer Clarke Group are searching for a community nurse on behalf of one of our clients for a 12-week contracted basis. The role is full time, part time also will be considered. We are looking for an individual to start as soon as possible. It is essential that the successful candidate has a Full UK driving licence and access to a vehicle.
Having knowledge in Dementia is also essential for this role as you will be working within the dementia assessment and support service. For this role in particular, you will be expected to travel over a wide geographical area, as there is an expectation to travel to undertake home visits.

Main responsibilities for the role:

  • You will take responsibility with the supervision along with the support from a Band 6 Nurse, for the management of an identified case load which includes, specialist assessment (within a specification), you will be responsible for care planning in conjunction with service users and carer.
  • You will be required to assess a service users care needs, along with developing, implementing, and evaluating programmes of care.
  • You will be expected to when appropriate to create risk assessments and risk management plans.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including pharmacological and psychologically informed interventions.


What we're looking for:

  • Registered Nurse, registered with the relevant body (NMC), can be both RGN and RMN.
  • Have experience working with dementia is essential.
  • Full UK driving licence is also essential for this role.
  • Experience of assessing, planning, co-ordinating and managing people with dementia and memory problems in either a community or ward setting.
