Spencer Clarke Group are seeking a Through-Care Navigator for a Local Authority Client in Berkshire.
In this role, you will support people who are rough sleeping or at risk of homelessness to access housing, health, and support services, helping them move towards a safer and more stable future.
Duties:
- Build relationships with clients and partner agencies to provide coordinated support.
- Develop and deliver personalised support plans to help clients access housing, health, and wellbeing services.
- Visit clients in the community, offering practical help to prevent or end rough sleeping.
- Maintain accurate records and work closely with the Rough Sleeping and Outreach teams to achieve positive outcomes.
Qualifications and Experience:
The successful candidate will have the following skills / experience:
- Experience supporting vulnerable adults, particularly those affected by homelessness or complex needs.
- Proven ability to develop and manage personalised support or action plans.
- Experience working collaboratively with multiple agencies and service providers.
- Strong record-keeping, communication, and organisational skills in a client-facing role.
What's on offer:
Salary: £20ph
*may negotiate higher for exceptional candidates, based on experience*
Contract type: 3-6 months minimum, with a high likelihood of extension
Hours: Monday to Friday, 37 hours per week
