Support Worker - Reablement/Planned Care

We are working with a public client in the Sunderland area to recruit several Support Workers for a Reablement at Home service.

This role involves supporting individuals in their own homes to regain independence and confidence following a decline in health. You will work within the community, helping people rebuild daily living skills and improve their quality of life.

Key Responsibilities

• 
  Support individuals with personal care and daily living activities
• 
  Promote independence, confidence, and wellbeing
• 
  Follow reablement plans and work towards agreed goals
• 
  Accurately record progress and report any concerns
• 
  Work flexibly across weekday and weekend shifts

Shift patterns include:

• 
  AM: 07:00 - 15:30
• 
  PM: 13:30 - 22:00

Key Requirements

• 
  Previous experience in care, support, or reablement is desirable
• 
  A caring, positive, and enabling attitude
• 
  Strong communication and written skills
• 
  Flexible and adaptable approach to work
• Ability to work evenings, weekends, and bank holidays

Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: Starting from £16.46.
Working pattern - 32 hours per week.
