I'm currently working with a public client in Herefordshire who is looking to appoint a Senior Social Care Assessor to join their Adult Social Care service. This is a great opportunity to join a supportive and forward-thinking team delivering community-based support to adults and young adults.

Role Overview


As a Senior Social Care Assessor, you will play a key role in supporting adults within the community, helping them maintain independence and improve their wellbeing. You will work within a multidisciplinary team including occupational therapists, social workers and partner organisations, completing strengths-based assessments and supporting individuals and carers to achieve positive outcomes.


This role offers the chance to work in a fast-paced and rewarding environment, with the autonomy to manage your own workload while being supported by an experienced and collaborative team.

Key Responsibilities

  • Undertake strengths-based assessments for adults and informal carers.
  • Complete mental capacity assessments where required.
  • Carry out care management and reviews to ensure appropriate support is in place.
  • Work closely with occupational therapists, social workers and partner agencies.
  • Support individuals to identify their desired outcomes and develop strengths-based plans to achieve them.
  • Maintain strong working relationships with service users, families, carers, advocates and external organisations.
  • Ensure work aligns with current legislation and guidance within adult social care.


Key Requirements

  • NVQ Level 3 or above in a relevant field (or equivalent experience).
  • Previous experience working in a care or adult social care environment.
  • Good understanding of adult social care legislation and guidance.
  • Experience completing person-centred, strengths-based assessments and care management along with care plans.
  • Demonstrable experience supporting individuals to identify and achieve personal outcomes.
  • Ability to build effective working relationships with service users, families, colleagues and partner organisations.


Contract Length: 3 months initially with the view of an extension.
Pay Rate: £18-£25 per hour | Negotiable DOE.
Working pattern - 37 hours per week
