SEN / SEMH Outreach Worker - Primary & Secondary

We are seeking a highly skilled, resilient, and self-motivated SEN/SEMH Outreach Worker to support primary and secondary-aged pupils across the Halifax area. This is a long-term role combining remote and on-site outreach work, supporting pupils who require alternative or additional provision outside of mainstream classroom settings.

This role is ideal for a confident professional with extensive knowledge of SEN and SEMH, who can work independently in the community while collaborating effectively with schools, families, and multi-agency teams.

The Role:

• Delivering 1:1 outreach support to pupils with SEMH and SEN needs across home, school, and community settings

• Supporting pupils to re-engage with education, manage behaviour, and regulate emotions

• Planning and delivering structured interventions tailored to individual needs

• Working both remotely and on-site, adapting approaches to suit each pupil

• Building strong relationships with pupils, families, schools, and professionals

• Maintaining accurate records and contributing to progress reviews and reports

• Working autonomously while remaining aligned with wider support teams

The Ideal Candidate Will Have:

• Extensive experience supporting pupils with SEMH and SEN

• Background in support work, youth work, care work, alternative provision, or education

• Ability to work independently, manage caseloads, and make professional decisions

• Strong understanding of behaviour, trauma-informed practice, and emotional regulation

• Excellent communication, organisation, and problem-solving skills

• Confidence working with challenging behaviour in varied environments

• A full driving licence and access to a car (essential)

• Strong safeguarding knowledge and professional boundaries
