Reablement Care Worker

We are supporting a client in Cardiff who is looking to grow their Community Resource Team (CRT). They are looking for a Reablement Care Worker, this is an excellent opportunity for caring, motivated individuals who want to make a real difference in the lives of people across the city.

You will support individuals in their own homes, helping to prevent unnecessary hospital admissions and assisting with safe discharge from hospital. Working closely with health professionals, you'll help each service user work towards their personal reablement and wellbeing goals.

Key Responsibilities

• Deliver high-quality homecare support based on each service user's personalised plan
• Work collaboratively with health colleagues to support reablement goals
• Record observations accurately and maintain clear documentation
• Work to a pre-planned rota, including potential evening and weekend shifts
• Use a mobile device to manage and complete allocated daily tasks

Key Requirements

• A caring, patient, and supportive approach
• Strong observation and recording skills
• Flexibility and the ability to adapt within a fast-paced service
• Willingness to train towards the required Health & Social Care qualification
• Ability to speak additional languages, including Welsh or community languages, is an advantage
• Must be willing to register with Social Care Wales
• Subject to an Enhanced DBS check

Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £14-£18 (Negotiable DOE)
Working pattern - 37 hours per week.
