We are working with a client in the Liverpool area who is looking for a dedicated Reablement Assistant to join their Adult Social Care team. This is a great opportunity to support adults in regaining independence and confidence within their own homes.


As a Reablement Assistant, you will support adults with day-to-day tasks, help them rebuild daily living skills, and encourage them to live as independently as possible. You will work as part of a multidisciplinary team to deliver personalised, person-centred support.


Key Responsibilities

  • Support individuals with their physical, social, and emotional needs as outlined in their reablement plan.
  • Assist with daily tasks such as personal care, meal preparation, and mobility.
  • Help service users develop or regain independent living skills.
  • Work alongside health and social care staff to reduce risks and maintain safety at home.
  • Follow and contribute to person-centred support plans, assessments, and reviews.
  • Maintain accurate records and report any changes to your Line Manager.
  • Support individuals with activities and community participation.
  • Follow all Council policies, including equality, dignity, and anti-discriminatory practice.


Key Requirements

  • Experience supporting adults in a care, reablement, or similar setting (paid or voluntary).
  • Good communication skills and a compassionate, person-centred approach.
  • Ability to encourage independence and work alongside other professionals.
  • Reliable, flexible, and committed to high-quality care.
  • Ability to maintain clear written records.


Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £14-£19 per hour
Working pattern - 37 hours per week
