We are currently seeking an experienced Leaving Care Personal Advisor to work with our local authority client in Cardiff. The role involves supporting young people aged 16-25 who are leaving care, helping them transition to independent living and achieve positive outcomes.

Key Responsibilities

  • Work with young people to create and review Pathway Plans.
  • Provide advice, guidance, and practical support around housing, education, employment, and health.
  • Help young people develop life skills and confidence to live independently.
  • Liaise with social workers, housing providers, education and training services, and other key partners.
  • Keep accurate case notes and reports in line with local authority policies and procedures.

Key Requirements

  • Previous experience working with care leavers or vulnerable young people.
  • Strong understanding of leaving care legislation and safeguarding.
  • Excellent communication and organisational skills.
  • Ability to build trusting relationships with young people.
  • Enhanced DBS check (or willingness to obtain one).
  • Relevant qualification in social care, youth work, or a related field preferred.

Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £16-£21
Working pattern - 37 hours per week.
