Outreach Family Support Worker - Early Help

We're recruiting an Outreach Family Support Worker to deliver high quality, home based Early Help support to children, young people and their families. You'll hold a caseload, provide targeted interventions, and coordinate multi agency support using a whole family, strengths based approach.


The role focuses on preventing escalation to statutory services and improving outcomes for children aged 0-25 (19-25 where SEND applies).


Key Responsibilities

  • Hold and manage a caseload of children and families with multiple or complex needs.
  • Complete Family Help assessments and lead Team Around the Family (TAF) meetings.
  • Provide intensive, targeted support to vulnerable children and young people, including those with emotional or mental health needs.
  • Develop and coordinate multi agency plans that reduce risk and improve outcomes.
  • Maintain accurate, timely case records, assessments, plans and chronologies.


Key Requirements

  • Experience working with children, young people and families with complex needs.
  • Strong assessment, planning and case holding skills.
  • Ability to lead multi agency meetings and coordinate support.
  • Excellent communication, organisation and report writing abilities.
  • Knowledge of safeguarding legislation and Early Help frameworks.


Contract Length: 3 months initially with the view of an extension.
Pay Rate: £15.87 - £20.87 per hour
Working pattern - 37 hours per week, Hybrid working arrangements.
