Experienced Occupational Therapist - Reablement
Location: South West England
Contract: Full Time
Sector: Adult Social Care
An experienced Occupational Therapist is required to join a busy Reablement Team within a local authority Adult Social Care service. The team focuses on supporting individuals to regain independence and reduce the need for long-term care.
Key Responsibilities
- Manage a complex caseload of adult clients
- Complete occupational therapy assessments and develop reablement plans
- Carry out moving & handling and risk assessments
- Recommend equipment, adaptations and Disabled Facilities Grants
- Work collaboratively with health professionals, families and partner agencies
- Provide professional support and guidance to colleagues
- Maintain accurate records and reports
Requirements
- Degree or Diploma in Occupational Therapy
- HCPC registration
- Minimum 2 years post-qualification experience
- Experience working with adult clients and complex needs
- Knowledge of health, housing and social care legislation
- Experience with adaptations, reablement and moving & handling
Additional Information
- Enhanced DBS required
- Travel required for home visits
