Leaving Care Personal Advisor

We are supporting a client in Sefton in recruiting a dedicated Leaving Care Personal Advisor to work with young people aged 16-25 who are preparing to leave, or have recently left, care.

You will play a key role in promoting independence, improving outcomes, and ensuring each young person receives the support, guidance, and opportunities they need to thrive.

Key Responsibilities


  • Provide advice, guidance, and practical support to care-experienced young people.

  • Take part in the assessment, preparation, and review of Pathway Plans with a person-centred approach.

  • Coordinate services and ensure young people access the support available to them.

  • Maintain regular contact with young people and monitor their progress and wellbeing.

  • Promote positive health, wellbeing, education, and independence skills.

  • Identify risk, safeguarding concerns, and escalate appropriately.

  • Work collaboratively with internal teams and multi-agency partners across Sefton.

  • Attend relevant meetings, representing the service as needed.

  • Maintain accurate and timely case records using ICS and other systems.

  • Support service development by contributing to meetings, reviews, and training.

Key Requirements


  • Experience supporting young people, ideally those who are care-experienced or transitioning to independence.

  • Strong understanding of safeguarding, risk management, and multi-agency working.

  • Ability to build positive, professional relationships with young people aged 16-25.

  • Good knowledge of Pathway Plans and legislation relating to care leavers (desirable).

  • Effective communication, record-keeping, and organisational skills.



Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £19-£24 (Negotiable DOE)
Working pattern - 37 hours per week
