We are working with a local authority in the North West area seeking an experienced Case Assessor to join their Adult Social Care team. The role involves carrying out assessments and reviews to support individuals in maintaining their independence and accessing the right care and support services.
Key Responsibilities:
- Undertake person-centred assessments and reviews under the Care Act (2014).
- Develop and monitor outcome-based care packages to meet individual needs.
- Work collaboratively with social workers, care providers, and partner agencies.
- Identify and report safeguarding concerns promptly in line with policy.
- Maintain accurate records and provide advice, guidance, and advocacy to service users.
Requirements:
- Experience working in adult social care or a similar setting.
- Strong knowledge of the Care Act (2014) and safeguarding principles.
- Excellent communication, IT, and record-keeping skills.
Contract length: 4 weeks - with expectation of extension.
Rate of Pay: £16.62 - £18.35 negotiable DOE.
Hours: Full time, 37 hours each week.
