Care Co - Ordinator (Level 1)

We are working alongside a respected care provider in Sunderland to recruit several Care Coordinators for their supported living services.

The service supports individuals with a range of complex needs, including Learning Disabilities, Autism, Behaviour that Challenges, and Mental Health needs. Care Coordinators will play a key role in ensuring customers develop skills, engage in their community, and enhance their overall quality of life.

Key Responsibilities


  • Coordinate and deliver high-quality, person-centred care across supported living settings.

  • Work effectively within a team and multi-agency environment.

  • Support customers to achieve independence through physical, emotional, and psychological support.

  • Provide intimate personal care, assist with health needs, and support daily living tasks.

  • Promote positive behavioural support to achieve positive outcomes.

  • Lead by example, upholding dignity, respect, and professionalism in all interactions.

  • Support team members and contribute to a safe, supportive, and high-quality service.

Key Requirements


  • Previous experience in care coordination or a similar role within supported living services.

  • Ability to work in challenging environments and support individuals with complex needs.

  • Strong values aligned with dignity, compassion, respect, and professionalism.

  • Commitment to delivering high standards of customer care and supporting independence.

  • Flexibility to work within set rota patterns as required by the service.

  • Accredited training such as Medication Handling, Moving & Assisting, or First Aid is advantageous.
  • Strong communication, organisational, and teamwork skills


Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £13-£17
Working pattern - 37 hours per week
